I'm Derek Beda. I've spent more than 30 years building and leading software teams in Silicon Valley, at Gap Corporate, AMD, PayPal, and a string of startups. Much of that time was in quality assurance and Agile delivery: the part of the job where you find out whether the process on the whiteboard matches what actually ships.

The same few problems show up on almost every struggling team. Too much work in progress. A definition of done that doesn't include testing. Plans built on hope. Meetings nobody would miss. Stakeholders who hear bad news late. None of these are hard to fix once you can see them clearly.
